Your intake packet must be completed prior to your first appointment.  Please complete them electronically using the portal.

If for some reason you are unable to use the portal, you may submit your paperwork using the following alternatives.

Print and complete your forms prior to your first appointment. Submit these using our website's 'File Uploader' or by fax, and we'll contact you to schedule.

You may be asked to complete additional forms. Find them here.

If you'd like us to be able to communicate with a third party about your private health information, fill out this form with the 3rd party's contact information, or download then upload a PDF copy.


Office Policies and Treatment Consent

You may be asked to renew your consent forms in order to see our doctors.

If Medicare is unlikely to cover a service (e.g. telemedicine), complete this form to let us know how you'd like us to proceed.
