Steps to register:
- Submit the form below. Within 1-2 business days, our staff will email you instructions to create an account with our online portal, Patient Ally, so you can complete the intake paperwork.
- Complete and submit the intake paperwork through the portal.
- Our staff will contact you by phone to schedule your first appointment, discuss insurance benefits, and answer any questions.
Note. If you have not received an email to create an account with Patient Ally within 1-2 business days, check the junk folder of your inbox for an email from the address firstname.lastname@example.org, or call (760) 650-2290 ext.2001 for assistance.
